Sample Questionnaire

We will be delighted to discuss our validated questionnaires with you - and usually come to your trust, clinic, surgery etc to explain how we work.

At this stage, we will, of course, leave you with a copy of our questionnaire and a sample feedback report etc for your detailed consideration. It has proven a source of considerable frustration for us in the past to publish sample questionnaires on this website  - as, believe it or not, individuals have copied the format for their internal use; a situation which helps nobody.
